1) NYS Covid-19 Vaccination Eligibility and Booking slots
Please use the below link to find the eligibility, slots availability and booking for the covid-19
Vaccine administered by NY state. IF YOU VISIT A LOCATION WITHOUT AN
APPOINTMENT YOU WILL NOT RECEIVE A VACCINE, So, appointment is required.

All HTS (Hindu Temple Society) COVID-19 vaccination slots filled up and HTS opened
the wait list sign up. We will call you on the first come first basis for any cancellation
slot. We have more people waiting than the allocation and our objective is to vaccinate
the maximum number members as soon as possible. We urge your cooperation and
understanding during these difficult times.

These members might a get call, if there are any open slots available for any reason.
Some of the appointments don’t have all the required information, which delays the
process. If you didn’t fill all the required information correctly then you must have the
correct information at the time of check-in. Please don’t bring any children to the
vaccination camp.
Document proof requirements:
For Johnson & Johnson vaccinations:
1. Paper copy of a government-issued photo ID or drivers license
2. Paper copy of your insurance card/medicare card
3. Paper copy of Consent form
For Pfizer vaccinations:
1. A government-issued photo ID or drivers license
2. Your insurance card/Medicare card
For wait-list appointments:
